Our mission : To Provide the quality care people deserve in the place they call home. Dedicated Professionals Companion Hospice’s interdisciplinary team includes the primary care physician, skilled nurses (RN, LPN), a certified wound-ostomy nurse, home health aides, therapists (PT, OT, ST), medical social workers, chaplains and bereavement support, trained volunteers, and unique to our program, music therapy. Providing Personal
Care The hospice team responsibilities include the management of the patient’s pain and symptoms, assisting the patient and family with the emotional, psychosocial, and spiritual concerns when dealing with end-of-life care. Companion Hospice also can provide medications, medical supplies, and equipment directly related to the terminal illness. We will also continue to advise the family on how to care for the patient as their condition changes, and will provide counseling and bereavement support for at least 13 months after the death of the patient.
